Highly tumorigenic lung cancer CD133+  cells display stem-like features and are spared by cisplatin treatment.

The identification of  lung tumor-initiating  cells and associated markers may be useful for optimization of therapeutic approaches and  for  predictive and  prognostic information  in  lung  cancer patients. CD133, a surface glycoprotein linked to organ-specific stem cells, was described as a marker of cancer-initiating cells in different tumor types. Here, we report that a CD133+, epithelial- specific antigen-positive (CD133+ESA+) population is increased in primary nonsmall cell lung cancer (NSCLC) compared  with normal lung tissue and has higher tumorigenic potential in SCID mice and expression of genes involved in stemness, adhesion, motility, and drug efflux than the CD133-  counterpart. Cisplatin treatment of lung cancer cells in vitro resulted in enrichment of CD133+ fraction both after acute cytotoxic exposure and in cells with stable cispla- tin-resistant phenotype. Subpopulations of CD133+ABCG2+  and CD133+CXCR4+ cells were spared by in vivo cisplatin treatment of lung tumor xenografts established from primary tumors. A ten- dency toward  shorter progression-free survival was observed in CD133+ NSCLC patients treated with platinum-containing regi- mens. Our results indicate that chemoresistant populations with highly tumorigenic and stem-like features are present in lung tumors. The molecular features of these cells may provide the rationale for more specific therapeutic targeting and the definition of predictive factors in clinical management of this lethal disease.

 

2009년 나온 이 논문은 조직 특이적 줄기세포와 연관된 표면 글리코단백질인 CD133 양성인 폐종양세포가 종양줄기세포 특징을 보이고 항암제에 더욱 저항성을 보인다는 내용으로 많은 보충자료와 실험들을 보여준다.

 

CD133은 prominin1으로도 불리며 여기서는 Flow cytometry와 Sphere-forming assay를 통해 세포를 분류하여 cancer stem cell의 특징을 확인하였다. 또한, 줄기 세포 특징을 유지하는데 중요한 역할을 하는 Oct-4의 발현 및 chemotherapy의 저항과 관련된 ATP-binding cassette(ABC) superfamily를 CD133+으로 확인하였다.

in vitro system과 사람 폐암을 쥐에 xenograft한 in vivo 실험을 통해 폐암에서의 CD133+가 종양을 생성하고 CSC 특징을 보이며, Cisplatin 처리에 대해 저항성을 갖는 것을 확인하였다. 

 

정상 폐 조직에서는 CD133+가 약 1% 미만이지만, 종양 조직에서는 약 5% 정도의 CD133+가 있었다.(56 중 47 샘플에서 확인 : 83.9%) 이 중 CD133+ 세포가 2% 미만인 샘플이 60% 정도이다.

 

 

 

 

 

위 그림은 IHC(immunohisochemistry)를 통해 종양을 보여준 것으로 parental tumor와 CD133+ 세포를 Xenofraft한 것이 거의 유사하다는 것을 보여주고 있다. 또한 FACS를 통해 CD133+ 세포의 %가 유사하다는 것을 한번 더 확인하고 있다.

 

 

 

 

 

위 그림은 Cisplatin 처리 후 CD133+ 양을 확인하였더니 처리 시 더욱 증가했다는 것을 보여준다. D에서는 생존도를 보여주었는데, CD133+가 생존 성에서 예후가 않좋은 것으로 보여진다.

이 밖의 실험으로는 primary, secondary, tertiary transplantation을 통해 CD133+ 가 종양을 생성할 수 있었다는 것과 분화가 더디다는 것 등을 보여주고 있다.

 

결과적으로 CD133+은 재발 등의 원인이 될 수 있는 종양줄기세포의 마커로 사용할 수 있다는 것 정도?
