Long-Term Recoveryof Irradiated Prostate Cancer

 

전립선 암은 개발도상국에서 가장 보편적인 악성종양으로 Prostate specific antigen을 통한 초기 발견과 치료 개선으로 5년의 생존률이 증가되었다. 그럼에도 불구하고, 전립선 암은 치사율이 2위인 암으로 미국에서 2010년도에 32,050 명의 목숨을 앗아갔다. 국지적 전립선 암의 처치는 수술과 방사선 치료가 있다. 20~50%의 환자의 경우 생화학적 치료가 실패를 경험하였으며 이는 재발을 가져오고 결국 죽음에 이르게 된다.

 

암줄기세포 모델은 최근 주목받고 있는데, 이 모델에 따르면 암을 시작하게 만들고, 유지하게 하며, 서브 집단을 구성한다. 암줄기세포의 특징은 자가재생하고 분화하며 결국 이질적 종양 세포를 생성하게 된다. 암줄기세포는 전립선암을 포함한 다양한 여러 종양에서 분리되었다. 이러한 암줄기세포의 마커로 CD44, CD133, integrin a2b1이 혼로 또는 콤비네이션을 이루어 이용된다. 암줄기세포는 높은 수준의 줄기세포 유전자와, 증가된 증식 능력, 자가 재생력, 종양생성 등을 non-CSC와 비교되어 발현한다. 더 나아가, 분리된 암줄기세포는 non-adherent 상태에서 sphere를 형성하여 자란다. prostatospheres는 전체 세포 중 0.1 ~ 8%의 거의 없는 세포로 부터 생성되며 또한 자가재생과 높은 증식률을 보여준다.

 

여기 실험자들은 LNCaP와 DU-145 전립선암 셀 라인 또는 환자샘플을 이용하여 prostatosphere을 분리하였다고 한다. 

다양한 조직의 CSCs는 non-CSCs에 비해 방사선 치료에 더 저항적이다. breast CSCs (CD24-/CD44+), Glioblastoma (CD133+), Brain CSCs (DNA damage checkpoint in response to radiation and repair radiation-induced DNA damage), Atypical teratoid/rhabdoid brain tumors(CD133+). 반대로 방사선 저항성은 CSCs의 일반적인 특징이 아닐 수도 있다는 연구가 진행되고 있다. brain, breast, colon, pancreas cancer에서 FACS에 의해 분류된 CSCs 중 오직 breast (MDA-MB-231)만이 방사선 저항성을 보였다고 한다. 그러나, 방사선 저항성은 전립선 암줄기세포에서 아직 보고 되지 않았다.

 

- Radioresistance는 DNA damage 나 변형을 통해 CSCs를 유도한다는 점이지 CSCs의 특징이다라고 할 수 없지 않을까? 

 

- 높은 방사선 조사 후 전립선 암세포는 다시 생성되었다.

재미 있는 부분이 일주일 안의 실험 결과 stemness 관련 유전자들과 cell surface maker 들이 감소되었거나 con과 같았다.

30일 후 실험 결과에서는 한 가지 정도 증가하는 양상을 보였다.

이는 방사선에 의한 데미지 회복 후 종양줄기세포의 특징들이 증가된 것을 나타나는데 중요성이 있다.

허나 실험 데이터 부분이 깔끔하지 못한 느낌이 있어서 아쉽다.

BACKGROUND. Despite improvements in treatment, prostate cancer (PC) remains the second-
leading cause of cancer death in men. Radiotherapy is among the first-line treatments
for PC, but a significant number of patients relapse. Recent evidence supports the idea that
PC is initiated by a subset of cells, termed cancer stem cells (CSCs). CSCs have also been
implicated in radioresistance in various malignancies, but their role in PC has not yet been
investigated.

METHODS. We compared the relative radiosensitivity of isolated CSCs to the total population
of their corresponding cell lines, and examined the relative numbers of CSCs in irradiated
cell lines following long-term recovery and in recurrent human PC.

RESULTS. Here, we show that while irradiation does not immediately favor increased survival
of CSCs, irradiated PC cell lines showed an increase in CSC properties with long-term
recovery. These data suggest that, although CSCs are initially damaged by radiation, they
possess a greater capacity for recovery and regrowth.
CONCLUSIONS. The combination of radiotherapy with a CSC-targeted therapeutic strategy
